New Patients

How to Register

To register with our practice you will need to complete a registration form which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

Please note all registrations are completed at our Blackbrook site on Lisieux Way.

After you have completed the form and returned it to the surgery you will be asked to come to the surgery with 2 forms of identification (ID) so that eligibility can be quickly confirmed.

We will need;

  • Proof of address, for example in the form of a recent utility bill.
  • Photogenic ID, for example a passport or a drivers licence.

Please allow two working days before coming in.

Medical treatment is available from the date of registration.

Although you will be officially registered with one particular doctor, it may be necessary to book you in with an alternative clinician. You may also request to see the doctor of your choice.

We do advise however, for continuity of care, you request your same Doctor for an individual problem so that they may get to know you.

Practice Boundary

If you live within our practice area you are welcome to register with us. Please check that you live within our catchment area.
